I would question that the rash is not yeast.  Ask the mother if she has
tried lotrimin on the rash instead of the cortisone.  The other thing that
comes to mind is diabetes in the mother.  Ask about her blood glucose
history.  I have a diabetic husband, and a close friend who both experience
this type of yeast rashes, and yes the cortisone creams do help, as does
staying on top of those blood levels.

SNIP......The weird thing here is that now the mother has a red raised rash
all over her breasts, not involving the nipple or areola. It itches, but
does not burn. Her OB treated it with steroid cream, and it went away, only
to reappear again after she discontinued the use of it.
