Very interesting discussion, indeed.
About the point of decreasing milksupply or not I can add that I and others
in my corner of the world do occasional use nippleshields in moms with an
overabundant milksupply where standard therapy in breastfeeding management do
not work (enough) or are not an option for the moher . The shields seem to
help decreasing the milkvolume to amounts that are workable for the nursling.
Also mothers with overactive milk ejection can benefit from "guided" shield
use as the milk will not be "bombarded" deep in baby's throat (sp?)
I also use shields with moms with nipples that can not be mastered by her
child, like very large ones or with unusual forms like big-oval, S-form,
8-form, etc. The shield may model the nipple into a form the child can latch
on to or rather that the baby can get past in order to reach the tissues
behind it. Time will clear this as baby's mouth grows.
