If this mom nurses the toddler with the cold sore, she runs the risk of getting herpes on her nipples.  If she develops herpes lesions on her nipples then she may transmit it to her 6 month old and possibly to a neonate if she nurses more babies.  An antiviral cream will not reliably prevent transmission of the herpes virus.  She is taking a  risk by nursing her toddler with a herpes lesion.
