So do her nipples look completely normal at this time? And they burn all the 
time or intermittently?

If nipples look normal and burning is intermittent, I would try heating pad 
on low right after pumping for 20 minutes, and as needed for episodes 
between pumpings. (presumed vasospasm secondary to pumping)

If nipples look pink and dry or scaley, and hurt all the time, I would use a 
steroid...may need a prescription now (0.1% triamcinolone OINTMENT) applied 
sparingly after every pumping until redness gone and pain much umproved. 
Then can decrease to 3 times a day until completely resolved.  Then may need 
to use 1% hydrocortisone ointment to keep it under control if she needs to 
continue pumping exclusively.

I doubt antibiotics are going to be helpful, inless there are large cracks 
that appear infected.
