We also care for many mothers on methadone.  I rarely recommend reglan for
anyone because almost all of our mothers are at high risk for depression,
but I would consider recommending it for any mother (including moms on
methadone) on an individual case basis.  Does she have a history of
depression?  Is she well connected to her health care provider?  Will she
understand that using this medication increases her risk for depression,
and that she should stop taking it if she begins to feel any untoward
effects?

In general, I'd rather help her get the baby to nurse more frequently, and
supplement that with hand expression and/or pumping in order to increase
her supply.  Many of our mothers have also had good luck with fenugreek.
