Is this really so prevalent in the US and the west? As far as I know
this is not done in the hospital where I teach childbirth classes, but
now I'll confirm that.

I have never seen a hospital in the USA where it is not done.  I don't have
access to the particular ACOG bulletin at the moment, but I believe it is
standard of care for all Ob-Gyns in the USA.  Is there an Ob-Gyn on LACTNET to
confirm?
