"I suggested charging the patient for formula - that bites into capitation -
but if we decrease intervention we are decreasing cost of services for our
moms and babies. This should not be difficult. Why is it? I feel a little
lame right now"

One of the policies at the hospital where I work is that mums who plan to
breastfeed are to bring in their own supplies - formula, bottles, teats etc.
Of course that doesn't affect those who start breastfeeding and then
introduce formula for whatever reason, but at least it's  a start.
I have the list of BFHI accredited hospitals in Australia, if that would
help
