I don't have the book in front of me...now, besides actually referring to  an 
OT or ST that does some oral training with breastfed kids...they do small  
feedings-controlled flow- to pace and encourage some feedings.  They (Wolf  and 
Glass) use a 'binky trainer' device...basically a paci and 5 french feeding  
tube and syringe to slowly pace and give feeds. But finger feeding would give  
the same control and pacing.  Also, some slow pressure on tongue while baby  
is sucking to encourage that down/forward movement.  Certainly these kids  are 
slow to learn, so lots of pumping for mom and lots of patience!
