Sorry this is so late, Jaye, I've been traveling/lecturing a lot and 
just catching up on LN. How about making the bottle progressively less 
friendly? Cold milk, only an ounce in the bottle at a time (then baby 
has to wait till it is refilled if he won't then take the breast), 
changing the nipple so it's one the baby doesn't love, making the baby 
suck for a few minutes before tipping the bottle up to put milk in the 
nipple, etc.
