What comes to my mind is, has anyone checked both
mother and child for poor nutrition?

eg, adequate vitamin E makes tissues such as
ovaries sufficiently elastic to handle normal
demands such as releasing ova, adequate B
vitamins and calcium combat irritability,
adequate protein and healthy fats make children
(as well as adults) feel full and satisfied after
eating, etc. Perhaps they could benefit from
supplements and a healthier diet.
