Diane,
I personally will suggest that mothers who are having 1st
trimester keep nursing to a minimum (only while they are bleeding)AND
that they completely refrain from intercourse...It seems to me that the
intercourse is likely to be worse because it would involve not only
oxytocin and potential uterine contractions (though it is debatable if
the uterus is responsive to oxytocin early in pregnancy) but also
prostaglandins in semin. I suspect the reason for her prior miscarriage
may be related to her older age (older mothers do have an increased
risk of miscarriage). I would recommend that she have an ultrasound as
soon as possible to evaluate if this pregnancy is viable...if it is
not - no reason not to keep breastfeeding the other (may even help her
psychologically)...if the pregnancy does have a heart beat then I would
restrict intercourse and have her nurse as little as possible until the
bleeding has stopped... Hope this helps! Pamela Berens
