Karleen, you wrote about a mom whi is adopting and pumping,

either). However, I discovered today that she's having a lot of pain when she 
starts pumping"

A couple of thoughts, has she tried nipple stimulation ala Jean Cotterman 
prior to pumping?  Nipple rolling, breast massage for a minute, then wait two 
minutes before pumping, which is what Jean recommends; 3 minutes until letdown 
occurs.  Also, homeopathy, such as Rescue Remedy or this remedy which is a 
homeopathic pituitary which a mom I worked with used and swore by and which a mom 
who currently has to tease out more milk from time to time and is using it with 
good results.  The site this latest mom is using to obtain the remedy is

homeopathic that might help can be found at 
http://www.kellymom.com/bf/supply/adoptivebf.html  and is a tincture.  

The last possibility is friction pain for your mom and a dab of olive oil 
rubbed into the flange where the tunnel meets the flared part can aid in smoother 
pulling on the breast.  Feel free to email me, Karleen, for further
