For Linda Klein who was looking for references with regard to breastfeeding lying down and ear 
infections:

This does not address the position of mother and baby while nursing but it is a great reference for 
the protection of breastfeeding on respiratory infections and otitis media: 

Pediatrics. 2006 Feb;117(2):425-32. 
Full breastfeeding duration and associated decrease in respiratory tract infection in US children.

The great thing about this paper is that it shows a dosage effect for breastfeeding.  Babies who 
were fully breastfed for 6 mo or greater had greater protection than those who breastfed for 4-6 
months, and so on -- showing the highest risk in the no breastmilk group. The greatest difference 
between the groups was seen for otitis media as compared to the other types of infections that 
they examined.
