I do not recommend the zinc-based ointment with GV in it for broken 
skin.  Seems as if the combination diaper cream was working.  One thing 
that I strongly recommend is that the Balmex or such zinc-based ointment be 
applied heavily as a a barrier to the stool which has yeast in it.

I have found oral Nystatin to be worthless in almost all cases.  The use of 
it just wastes time and allows an expanded entrenchment of the yeast. I 
have had mothers who are using it and find that the baby's thrush gets 
worse.  It may be the sugar in it or it may be from contamination of the 
suspension from putting a yeasty dropper back into the bottle. I instruct 
them differently but they often call me late in the case.

When the skin is not broken I do recommend the added GV if the ointment is 
not showing improvement.
