Kim, I also work for Public Health as the home visiting manager.  Since both the doll and the breast model are items that we hold and not the mother, and we use hand sanitizer prior to the visit, I just wash the onesie on the doll periodically and spray her with Lysol between.  The breast model stays in a plastic bag and doesn't really get that dirty although it also gets a spray.  I would think the soft breast model could be put in a lingerie bag and tossed in the washer periodically.

Just a thought.
