Have you thought of checking the lingual frenulum?  Some babies with
tongue tie will have a reverse peristaltic-like action of the tongue
(back to front). [Reference: Fernando, Carmen. Tongue tie: from
confusion to clarity. - See my review in the Dec. 1998 JHL.]  If this is
the case, and not a developmental lag, it may take time to re-educate
for correct suck after correction of the frenulum.
   However, you seem to be doing some good things, resulting in some
progress, and the mother seems well motivated, which sure helps.
