Dear anna,
this baby sounds very ill and may have severe cowsmilk protein allergy
(especially if parents have allergic history) but should also be worked up
for other things like cystic fibrosis, urinary and intestinal infections and
others.
Soy was definitely the bad choice since it is felt that many if not most
babies allergic to cows milk are also allergic to soy.
Anyhow breast milk may save this baby in many ways.  It would usually take a
week to get a weaning mother to reestablish her milk supply this baby can
take 3 or 4 times that to get the mother to produce enough milk.
Although it may not be easy since this baby has not been at the breast at
all since the age of 4 days (or has (s)he?)The ideal would be to get the
baby to feed at the breast using a lactaid or homemade accessory milk
dispenser while nursing. You can get help from fellow consultants if need be
and wean the dispenser gradually over 4 to 6 weeks if the baby gains enough.
If the baby won't nurse,keep trying regularly at times when (s)he is not too
hungry and use bottles in the meantime or better cup feeding. A full cows
milk protein free formula must be used and can be enriched with a medium
chain triglyceride oil providing it is does not contain peanut or soy oil.At
the same time the mother should be helped to express milk by hand or pump,
at least 5 times a day (preferably during feeding of the other breast) and
feed that to eliminate formula feeding as soon as possible.
This baby should have no fish till the age of at least 3 years and no eggs
till the age of 2 years.
