I have several ways I chart and I chart everything.

First of all, all moms fill in a teaching checklist upon admission to
the unit, one of the sections is breastfeeding teaching, there is a
section for the nurse or LC to chart the date and sign that she provided
the teaching the pt requested.  I try to see every bf mom, even if she
didn't mark that box, I sign anyway that I instructed her.

We also have a breastfeeding assessment form/checklist. It is one full
page and has room for several assessments. BF assessments are to be
charted once per shift, by nurse or LC and i made this form to make it
easier. If there are no problems identified on the checklist, then no
note is made in the progress record, if there are problems identified,
then a note is made in the chart.

I make telephone callbacks to ALL bf moms, the reverse side of the first
form I described is designed for this, it has questions to ask and a
place to chart moms response and lines at the bottom for extra notes.
When mom is d/c from the hospital these forms are placed in a special
folder until the call back is done and then I send them to medical
records and they are a part of the permanent chart.

Hope this is helpful, I always go by the rule, if you didn't chart it,
you didn't do it.
