Unfortunately this happens a lot.  Its amazing people find it acceptable to giv 
a bottle of formula to a baby against standard bf orders, then not document.  
Can you imagine if as often as a baby was given an undocumented bottle,  
nurse were to give a pt a medication against orders, bc its what they thought 
the patient needed? All on the sly, no documentation? It is completely beyond 
the scope of practice and reportable to OPD.  They would be in severe danger 
of losing there professional license.  If anyone is interested there is an article 
in the Journal of Human Lactation, 16 (1), from the yr. 2000 called "Using the 
Law of Battery to Protect Breastfeeding." It addresses these feedings  and 
the possible legal ramifications to the health care personnel that administers it.
