I want to be sarcastic and ask if the moms have no breasts :-b  True lactation insufficiency is rare and many of those moms have visual clues when you look at the breasts. 
 
But frankly that is not the idea I'd want to put in the staff's minds.  What I'd want to tell them is that almost every mom makes milk and lactation is a process, beginning with colostrum.  Removal of the colostrum by a baby learning to nurse is the trigger that tells the brain (pituitary)" turn on, make milk!"  The longer it takes for the brain to get the message, the less successful lactation duration will be, so early and often removal of colostrum is an extremely important function.  By "topping up" with formula, you delay the whole process and possibly begin the slide down to formula feeding.  Moms need the confidence boost of a support person telling them that frequent removal of colostrum is exactly the message that baby needs to give the breasts.
 
Maybe these WICU staff need a short course on how lactation happens. And one on physiology of baby's gut - size etc. There is an  excellent outline in  The Curriculum in "Support of the Ten Steps to Successful Breastfeeding" -  (BFHI)  Pat in SNJ
