I just worked with a mom that sounds pretty similar.  This baby had the 
largest blisters at 4 weeks of age across the entire top lip.  I have never 
seen anything so extreme.  He was gaining beautifully.  Mom was in a lot of 
pain, latch looked deep, but was he was moving his lips to much.  With him, 
it ended up being frenulum issues, mainly the labial frenulum- he could not 
really flange properly, and I guess all the friction was causing the 
blisters.
I agree to check
1. frenulum
2. I find CST very helpful in these situations
3. I agree to keep the plan simple.  I am a big fan of pumping and using a 
bottle- in this case I would choose the "hardest" one which I find is an 
Avent- assuming he does not do fish lips there.  But I have found the 
mothers can work them towards a big "latch" on the bottle gradually.
