To Kris: I don't know of any guidelines or ratios for outpatients per day per IBCLC. I am a hospital-based LC and we allot an hour for a consult as a basis for what we charge but I find they usually take longer (patient is late, extra time needed to rouse and then settle baby, paperwork, etc.) and then there is the time it takes to chart. My experience suggests that the consult itself may be an hour but it usually is closer to two hours of time invested.

To Stephanie: Does the FDA say why? Methergine has a half-life of only 30 minutes and peaks anywhere from 1/2 hour to 3 hours. Hale also says the baby is getting only a relative Infant Dose of 2%, which is quite low. (Hale's book states that many researchers feel anything less than 10% is probably safe.) Suggesting not nursing for such a long period seems contrary to this evidence, unless there is some new information.
