There was no specific  mention of drastic positional changes to achieve
moving the jet of milk away from the gag reflex point. The impression I
received was that even slightly changing the aiming of the nipple in the
mouth, different from whatever the attachment was before, (more
assymetrical, symmetrical)  was sometimes enough to improve the
situation. I cant see why positional changes would not also have this
effect..worth a try!
