thanks for your response...yes it is an interesting case.
The mother is a fairly average weight and height, approx 5 ft 6 and 60-70kg
when I saw her postpartum (with the first baby....the new baby is due in
Feb)
Yes, the breasts grew approx 1 cup size and they were incredibly tender from
day 1 (however my input only started at 4 weeks pp)....I gather this
discouraged her from feeding and expressing and she spoke of giving large
volumes of artificial milk from very early...she said there was no obvious
milk and very little swallowing. There was no dramatic change in the breasts
3-4 days pp.
And at 4 weeks postpartum, I watched baby latch beautifully but the swallows
were minimal.
Yes, with such high prolactin one would have hoped for a better result....so
I was interested when a couple of other lactnetters plus the lab result
sheet mentioned the possibility of prolactin binding to something like an
immunoglobulin and maybe adding to the problem.
Or do you think if this mother had been assisted better in the initial
weeks...there would have been a better outcome?
We really just want to get the best result possible this time and be
realistic in our approach...she is seeing her endocrinologist next week so
we will  ask her as well.
