You're right, but I can just see myself writing an order like that- when they
have standing orders that are prewritten so most docs are not 'composing' their
own orders- and the RN's complaining about me to someone superior to me- I
would have to be very sure that the parents wanted it that way too- and a few
parents that I prenatal consulted never chose me as a pediatrician- I think
maybe because I pushed breastfeeding too hard :(  The RN's in this hospital are
allowed to decide things like when to order Bili- so I don't think they would
appreciate having to "inform" me before giving formula- especially when they
(this was the night shift) told me that they think "Breast AND Bottle is the
best!"
