I don't work in a NICU, and I am sure you will get some responses from those 
who do. 
You swithch from colostrum to mature milk as it becomes available.  Feed the 
baby the milk in the order in which is was expressed.  It is a natural 
progression from colostrum, to tranisitional milk to mature milk.  Please keep in 
mind that these are all external  constructs that we put on the milk.  It is 
important to get it all; and its easy, as it comes out in the correct 'order'.
