This Cochrane Review (as many of the like) uses a very narrow minded view in order to create a sense of ultimate ''scientificallity'' (is that even a word?). By doing so, all kinds of factors are erased as being interfering with objectivity. And the sole thing that is looked on is numbers. None of those results have to do with quality (of breastfeeding, of experience, of life), just quantity. They don't look at how breastfeeding was going before pacifier use, what pacifier is used or how it is used, how time spent at breast relates to time spent with pacifier or other comforting methods, reason for advising or starting the pacifier, and on and on. 
Bottom line with pacifiers is that they tend to physically split the mother-baby unity, reduce the time baby is in arms and at breast and that that is not for the better for mom, baby or their relation and not for breastfeeding sustainment and satisfaction. But how could any study ever research that?

Yes! Thank you. As a result of the mPINC scores, the DHS is actively helping a great number of hospitals in my state to improve BrFdg rates and outcomes, they are teaching 5 of the 10 BFHI Steps. Obviously, one is the use of pacifiers. With this article they are saying now, "Why is it that we need to discourage pacifiers? This Cochrane Review says pacifiers do not affect BrFdg rates or duration."
Many staff feel the only real effect of avoiding pacifiers is to make life difficult for RNs and new moms who are tired of BrFdg "all the time". Well, you already know that drill. I'm preaching to the choir on this list.

My thoughts on pacifiers is:  if the study does not control for the TYPE of pacifier used (shape and length) with consistency between the groups, it is not a valid study.  In my experience, many of the babies with breastfeeding problems are using flat or weirdly shaped pacifiers that encourage biting and not sucking.  It's like comparing bottles and breastfeeding - it all depends on which mom, which baby, which bottle, etc.
