I am not an OB or Midwife, but this is clearly a situation where she should be educated about the fact that breastfeeding DOES cause spikes in Oxytocin, which she can see IS increasing her contractions, therefore it would be wise to NOT breastfeed until her twins are safely born as close to term as possible.  She can always resume breastfeeding the 18 month old at that time.  Once she understands that she is putting her new babies at risk by being born earlier than the need to be, if she chooses to forge ahead anyway, I do not think you can forcibly restrain her.



MOST moms can nurse through a pregnancy without any trouble, but MOST moms are not in the hospital with preterm labor and twins.  We do not have much research on it, because there are so few moms to study.



For what it is worth, you can tell her a breastfeeding medicine specialist would recommend she stop breastfeeding for now, and resume when the babies come.

Subject: preterm labor and breastfeeding



I think this is a question for any OB's and Midwives out there.  Pt. is 32 weeks pregnant with twins.  She has been admitted to labor department with preterm labor and is being treated with mag sulfate.  Her 18 month old was visiting her and breastfeeding which caused an increase in contractions.  This is being reported to me, I do not have access to the monitor strip.  The staff told her that she should not breastfeed.  This is upsetting to her and although they have explained the rationale why not to breastfeed, still chooses to breastfeed.  She says everything she has read says it is ok to breastfeed in pregnancy and that she would have lost the pregnancy before now if that were the case.  Does anyone have any literature or case studies (recent, evidenced) on high risk pregnancy and breastfeeding?
