I suspect this is a way hospitals "pass the buck" when challenged as to
whether both credentials are required.  I know there are IBCLCs working
in hospitals that are NOT also RNs.
 Physical, Occupational and Speech therapists  and Social Workers aren't
required to be RNs also!  I think the problem is that they don't quite
know where to put us.  Also, the lack of a specific, consistent
educational program in prep for becoming an LC may put us at somewhat of
a disadvantage.
I only sign IBCLC after my name when charting at our hospital.  When I
feel the need to "impress" higher-ups or to shortcut to being accepted
professionally without taking the time to explain the background to be
IBCLC, I will also add BSN and/or RNC.  However, I feel when functioning
as LCs our IBCLC should be the only criteria.
