In my experience, I do NOT ever buy directly from the publisher (especially the big ones), especially at conferences. I look at the books at the conference and then go buy them for cheaper via Amazon and with the knowledge that I can get a refund with no problem should something go awry, such as a faulty access code.  I learned that the hard way.  I've had to buy lots of textbooks and lactation books with access codes, and on occasion they have not worked.  Amazon has ALWAYS refunded or exchanged my book, no questions asked.  I know that is too late to help now, but may be useful in the future. 

Shame on the publisher (at the conference) for 1) selling a copy of a book with a used access code (those should have been all NEW books) and 2) refusing to refund or exchange your book.  That is just plain bad business...and why I won't buy directly from them.
