Jan,

I'm sure you've included circumcision in your list of risk factors--that's MY
current soapbox. A disproportionate number of boys get discharged nursing
poorly because they've been subjected to yet one more trauma. We must have a
circumcision rate here of >95% so it's no wonder we need to do so much
follow-up.

Circ's are normally done when convenient for the physician, but if the mom has
had a C-sec and baby is sleepy/drugged/not nursing yet we can usually talk the
doc into waiting until closer to discharge to do it. It's the vaginal births
that cause the most difficulty. Occasionally we suggest that the parents ask to
have it done in the office later when the baby has had an especially traumatic
birth, and some of the docs will do this. Does anyone work where it is
*routine* to wait and have it done on an out-patient basis--when baby has
recovered from the birth and is nursing well? It seems like it would be a good
compromise if parents are going to do it.
