In some cases, a shield may help if all other avenues have been
exhausted.  You don't want to risk baby getting "hooked" on the
shield and have to deal with that problem unless there is no other
alternative.
You didn't mention if she has tried different positions.  I find
that if you can aim any injured area of the nipple or areola toward
the side of the baby's mouth rather than toward the top or bottom of
the mouth, there seems to be less irritation of the injury while it
heals (and whatever caused it is corrected).  Sometimes this may
require some creative holds, but I think it would be worth a try.
In any case like this where a mom is having a problem with one side,
especially if she is or might be "resting" that side for a while,
moms worry whether one side can be enough.  I like to point out that
the "bottom line" (or worst-case scenario) is that it is possible to
nurse totally on only one side. I don't anticipate that that will
have to be the case for her, but I reassure her that it's not a case
of "all or nothing" and that she won't have to give up nursing if
the problem on the one side doesn't resolve.  Many times, just
knowing this helps the mom to relax and enjoy nursing while she
works on and usually overcomes the problem.
