Hi!  My name is Linda Moore, Rn, IBCLC.  I work in a hospital setting.
Our hospital is working on an "informed consent" form for a parent to sign
if formula is to be given to a breastfeeding infant.  Is there a resource
or has someone already invented this form?  Thank you for your help.

The discussion re: physician order, parent preference, and formula has
