This would not be the advice of any neo I work with. The infant would be encouraged to breastfeed on demand with test weights after mom's milk comes in. Supplement would at the breast with an SNS based on a standard formula of ml/kg/day based on days of age until weight gain was appropriate (15-30 g per day). These babies may need extra calories as they do not have "the packed lunch" on board that Jane Morton from Stanford talks about the healthy term infant. This is if there are no other health issues with the baby. If a baby is compromised, he may need to be tube fed until oral feeding can be initated safely. With these moms I always begin a pumping regime in addition to direct breastfeeding as the IUGR and SGA sometime does not have the energy to provide the type of nipple stimulation and is unable to empty the breast well enough to establish a robust supply.
