Jennifer:  In chronic cases of ductal yeast, I have rarely found topicals to
be sufficient (necessary, yes, but not sufficient).  I've posted on this
before, but here's a briefing on it.  If the moms cannot or don't want to
obtain Diflucan, my treatment of choice is citrus seed extract (aka
grapefruit seed extract).  The protocol I am using in my study is 10-15
drops in capsule form (we are attempting to blind the consumer to which
treatment they have and the taste is a dead giveaway--powdered extract is
not as potent, so this is our compromise), four times per day for 21-28
days.  That is stage one.  Either concommitently or following this, the moms
must use a multi-strain lactobacilli supplement with FOS for at least 8-12
weeks (some longer if they had been on long-term antibiotic therapy).  Other
supplements may be necessary if there has been substantial damage to the
intestinal wall (leading to leaky gut syndrome).

One point that some people miss with yeast treatment is warning about the
yeast die-off reaction.  Once the yeast cells start collapsing, the patient
may feel worse for a few days.  This may mean anything from mild headache to
full-blown fluish body aches.  It is not the medication (Diflucan and
Nizoral cause this as well if there is massive die off).
