(Donna asked about when the nurse - pump - supplement scenario was
appropriate and when the nurse, nurse and more nurse might be more
appropriate.

I often say that you can never go wrong nursing, you can never nurse too
often, etc. As an LLLL, I think that's the best advice you can give, the
first course of action and so on.
In my experience (personal and helping) a baby who is not getting enough
at the breast will let his mother know my coming off or going off and on
and generally being miserable and possibly sickly or lethargic. There
are really good indications in the RBAB about when a baby might need a
supplement and how to come up with a workable plan with the mother.
Often the scenario you describe sounds complicated, but it probably
isn't a lot more so than having a miserable baby not latched on properly
and not thriving. I encourage the moms to see this as a short-term
investment of time... something she's doing while she's fixing the
latch. And hour-long feeds happen with healthy, unsupplemented newborns
too. I always wondered what the big deal was... what are you supposed to
be doing with the baby when you're not sitting breastfeeding anyway?
Like, he's not sleeping, is he? So the choice is, hold him nursing, hold
him not nursing, carry him around nursing, carry him around not nursing.
MMM.... At least when I'm breastfeeding I have an excuse for not getting
up!
