hi hi---from a totally non-medicalised point of view this is common during tandem nursing during pregnancy and when a child is weaning... essentially baby is nursing on an "empty" breast... suggested management is when it starts to hurt ask toddler to stop nursing and they usually do because hurting =no milk Once mother shortens the nursing sessions the afterpains shouldn't be bad (if existent at all)... IMO these are the first signs of toddler losing ability to nurse/stimulate breast/milk production slowing down--I. E. natural weaning!! some mothers find that with just one nursing session its hard to maintain a good milk supply.. but worth looking at anti-galactogogues taht are in her diet, or adding some galactogogues... also, if she's letting baby nurse for long it could hurt throughout the day because its really hard on the breast!!
