In updating assessment and education hospital paper charting for nurses, we are discussing where to document what information: mother's chart or baby's chart. Baby care teaching and assessment might be documented on baby's chart, BUT breast assessment, breast care and breastfeeding teaching may be written on mother's chart? Why divide different aspects of the whole? It is the mother's body BUT breast assessment, care and education are relevent to breastfeeding the baby. Duplicate paperwork? Would this result in multiple charts to search? More time for LC or Pediatrician wanting to know what has been taught? Does anyone have a solution that they are happy with? Is this question show a continuation of the thinking in the old hospital seperate model? OR since they have seperate bodies, should they have 2 charts?  Does anyone use one chart for both mother and baby in a hospital? How does it work in computerized charting?
