Well, I do think that if there's any doubt at the time the shield is issued
as to whether follow up will be able to occur, then its best not to issue
the nipple shield in the first place.  IMO they MUST be followed-up, or at
the least the mom should go home with a good referral in her hand, with
instruction to obtain follow up from another LC in the community..
