Another thought...

Babies with oral challenges often compensate with a really strong suck and this could cause mom pain.  Also, mom being used to that strong suck, may be using too much suction with the breastpump.  She could try gentle massage and RPS to stimulate letdown and then try using a lower suction on her pump, making use of hands-on-pumping techniques.  Breaks from the pump, between let-downs, to use more RPS could also be helpful.
