You are on the right track. You just have to be very specific. For example:

# of wet diapers in 24hrs - ask how heavy the diapers are and the color
should be clear. Dark yellow, tea color, brick dust is a red flag
# and color of stools in 24 hrs - I like to see 4+ at least teaspoon sized
stools per 24 hrs. My coworker says 3 per day of tablespoon size.
# of feedings in 24 hrs - are the breasts softer after, is baby swallowing
during? how long? is baby content and sleepy pc.?
length of feedings - again need to know if milk transfer is taking place.
short or long feedings (strictly time-wise) dont' really tell you alot.
any supplements - ok
nipple soreness - yes, I ask about pain (I use 0-10 scale and I dont' like
any 4's or over), bruises, cracks, blisters, scabs, bleeding
breast fullness - yes I want to know if "milk came In" and what happened
after that, like is baby latching worse, is engorgement resolving, etc.
Babies getting milk are content pc and dirtying diapers alot.
audible swallowing - yes; do parents know how to assess this? have they been
shown prior to d/c.
If these things are not occuring, give advice, and call the next day and the
next - or if you really think they need to come in then bring them in. Like
if the mother has pain while nursing, cracks, and few dirty diapers they
need to come in. This is the gist of it anyway.
