It sounds like this baby has a tongue tie.  WHen a tongue tied infant
attempts to extend the tongue, it either won't budge, or the tongue tip
rolls down over the lower lip.  He might have a tight frenulum that is
not attached all the way to the tongue tip, but is still retrictive.
See if the baby can lift the tongue  tip to touch his palate when his
mouth is at least half open.  If not, there might be restriction.

Even a minor degree of tongue tie can reduce a baby's ability to pace
feeding.  Pair that with a mom with a strong MER and you can have one
miserable baby, who clamps or goes shallow to reduce the feeling of
drowning.
