I have posted about what is called nipple confusion many times, and
recommend everytime I do that we standardize the definition.  There is a
proposed medical def. developed by Neifert, Lawrence, and Seacat in J Peds
1995; 126:S125-29.

I saw a baby yest. who was two weeks old and had been exclusively bottle fed
pumped milk the entire time by a mom with very flat nipples.  The two weeks
of pumping had successfully drawn out the nipples some, and we had
absolutely no prob. latching this baby on for 3 reasons:  1) He was not
starving but was plump and energetic and could help us  2) the nipples were
more elastic, thus the problem that made the breasts 'not work' was
resolved, and 3) the pumping had protected the milk supply so we had a full,
rewarding breast to work with.  We put the baby in a feeding position he had
no negative assoc. with (side-lying) pinched up the nipple in a teacup hold
to give him a bit more tissue to sense, and he latched and took two oz out
of each breast, looking very relaxed and pleased with himself.

It is not nipple confusion if the baby can't make it work.  Our job is to
discover what doesn't work and attempt to fix it.  In the meantime you
protect the baby's growth and energy and protect the milk supply and you
don't plant negative suggestions about fatal nipple confusion.  When the
baby can make it work, breastfeeding proceeds normally.  If a baby can't
make it work there is something abnormal going on -- either with the breast,
the baby, or both.
