The bunched-up fabric under the breast really does support a good
amount of the weight and helps get the nipple pointed in the direction
of the baby.  This is quite comfortable to do and I speak with
personal experience.  Very large size bras can be custom made for a
fairly resonable price, I will be happy to supply info on the
American company if anyone wants to email me privately.
