I wonder if the early appearance of milk was what would happen naturally
after delivery, then without on-going nerve stimulus to nipples (since
nerves were severed during surgery) the brain didn't get the correct
signals and breasts gradually stopped making milk.  ???? I know the
autocrine/endocrine system should have kicked in here somewhere, but I
wonder about the disruption of nerve signals to the pituitary.  I think the
gain at one month of 11 oz was first major red flag.  If BF is going well
that is a barely acceptable weight gain of 11 oz for 4 weeks.  The gain of
12 oz in next 2 weeks would have been encouraging.

The other aspect is baby factors.  Are you totally sure the baby is well?
No heart problem or UTI or diabetes insipidus
(that one happened to one of my pts, but that baby just gained poorly all
along)  ?  Babies just don't lose major amounts of weight (23 oz) like that
in 3 weeks even if supply is dwindling.  Baby would have responded early on
with increased need to nurse, then as weight loss increased baby would
become apathetic.

It may be a combination of factors, not just one thing (ie:augmentation.)
but personally I think the augmentation was a major starting point for this
mom's problems.  I don't agree with the local professionals.

At this point the need is to feed the baby.  Baby got immunities etc. for
first 9 weeks and even with this major set-back I would hope that that will
count for something in baby's overall general health later on.  This mom
will need some help adjusting to the fact that her dream of nursing is
shattered.   A woman who had breast augmentation in the first place
probably has some issues to deal with about her body image and function.
This unfortunate outcome just confirms her opinion of her inadequate
breasts.
