I think the calcium numbers from the Harriet Lane handbook are in error.

According to the PediaSure website
http://www.pediasure.com/thisispediasure/nutriinfo.cfm , it contains
230 mg calcium in 8 oz, or ~29 mg/oz.

According to Appendix A in Ruth Lawrence's _Breastfeeding: A Guide
for the Medical Profession_ (p. 739), mature breastmilk has 28 mg
calcium per 100 ml, which works out to ~8.3 mg/oz.  So theoretically
a 13 month old would need to consume 60 ounces of breastmilk per day
to meet the RDA!

We know that there are many healthy babies this age who still get the
majority of their nutrition from breastmilk, and we know that
breastfed babies grow up to have better bone density than
artificially-fed babies, so these numbers don't tell the whole story
of course.
