Update: Baby continues to get a shallow latch (mid-tongue tie) but
doing well with nipple shield. At the end of the last feed, nipple was
nicely pulled into the shield and there was milk in the tip too. Mom
pumped 40 cc last evening, then she's gotten 20 each time she has
pumped since. Palpable alveolae in outer quadrants when I checked this
morning (~56 hrs postpartum). Mom and dad had a meltdown this morning
but things have been MUCH better since baby has been supplemented with
mom's own expressed milk. Also gave her a copy of a great article
about a 70 year old first-time-mom which Sonya Myles sent me
http://www.guardian.co.uk/lifeandstyle/2009/mar/06/fertility-problems-india

Plan currently: mom to palpate breasts before a feed, and again
after. If baby has drained the breast well, hot dog! If not - pump.
The baby's weight loss TOTAL was 12% this morning but only 4% since
the 24 hour weight - pedi not concerned enough to even grouse. Hoping
to slowly but surely pull back on the pumping. I want to compliment
the two RNs who have been working with her - Lyndsay Cunningham and
Meghan Dockum. A blessing to have the continuity and the faith in
breastfeeding so mom feels supported, not badgered.

Dawn Kersula...not quite so worried in Brattleboro, VT
where the Birthing Center Nurses Rock!!!
