I'm wondering if the mom in Annie's case needs to be on the
antibiotics for a longer period of time.  She first saw the mom at
10 days and it sounds like she had already had the course of
antibiotics and the mastitis had returned which means she hadn't
been on it very long.  I wonder if it was the short-term drug
Zithromax which has, if I remember correctly, only 5-7 days of Rx.
If one antibiotic hasn't worked, maybe she needs a different one.
