Problem:   I have a patient with a 6 month old solely  breastfed, she is now
pregnant again- she now knows BF is not a form of  birth control- Problem is
baby will not take formula at all in any way  shape or form.  Baby is onsomon
some food and is healthy and well  nourished. 


I am confused. If the baby is solely breastfed, that means the baby is only  
getting breastmilk. If the baby is on some foods, that means the baby is only  
partially breastfed. The contraceptive benefits of breastfeeding is highest 
if  the baby is totally breastfed at the breast. If the baby is getting some 
food,  the contraceptive effects diminish. This is best explained to the mothers 
early  in their lactation experience. Even babies given pacifiers to sleep 
may sleep  longer periods and diminish the contraceptive effects of 
breastfeeding.  Contraceptive methods, including the effects of lactation on fertility 
should be  included in prenatal classes, by a professional in that field. 
I noticed that you are not an IBCLC. You may want to bring one onto the  team 
working with your clients. That way, your clients are getting the benefits  
of the nutritionist, counseling them on nutrition, and the lactation  
consultant counseling them on breastfeeding management. If you are interested in  
becoming an IBLC, the best way is to be working with one as your mentor. If your  
supervisor is not interested in having an IBCLC on staff, it can be difficult 
to  provide all the services that your clients need, especially up to date and  
accurate counseling. Also, you may need to check with your liability 
insurance  to make sure you are covered for providing lactation services.
