Sorry, not my idea of a supportive physician.  The baby should be on breast.
If this does not work, put on expressed milk by alternative means, even
bottle.

By the way, here is a perfect example of thinking bottle feeding.  You have
a bottle feeding baby spitting up to the point of not gaining weight.  What
to do?  Short frequent feedings.

A breastfed baby?  Short frequent feedings?  No!  You allow the baby to fall
asleep on the breast, thus keeping him there as long as possible (not
necessarily feeding all that time--that's a different issue).  Why?  Do
babies spit up while on the breast?  Sometimes, but very uncommonly.  So
while the baby is doing non nutritive sucking, the stomach is emptying, and
thus the baby spits up less.  This I considered likely in theory, and now I
have a few cases where it worked.  Ha!
